I've done the same thing in Zero Mission. Sometimes I just forget a pick-up.

This kinda stuff usually happens when you know the route too well actually, at least to me. I've played through Zero Mission so many times that the 100% route is routine. Every once in a while I forget something and don't notice that my missile count or something is off until much later because I just expected myself to have done it.
