If you can make it around the Metroids in Tourian, there doesn't seem to be much that stops a 1-item run from being feasible.

I know for certain that there's a TAS of it.  As for why people aren't running it, perhaps people are aware of how boring it may be, so choose not to do it, or maybe they just don't see it as a speed run, as such, which this site is aimed at...
